📊 Actionable Insights for Every Resident

America is aging fast.
Our approach to senior care has not kept up.

By 2030, one in five Americans will be 65 or older. That is 71 million people. And yet for most of them, functional decline goes unnoticed until something goes wrong. A fall. A fracture. A hospital stay. A loss of independence that did not have to happen.

1 in 4
Adults 65+ fall every year
Over 14 million older Americans fall annually, yet fewer than half ever tell their doctor.
43,000
Deaths from falls in 2024
Falls are the leading cause of injury-related death among adults 65 and older, and the death rate has risen 21% since 2018.
$80B
Annual healthcare cost of falls
Fall-related care costs $80 billion a year and is projected to top $101 billion by 2030 as the population ages.
3.85M
ER visits from falls in 2023
Emergency department visits for fall-related injuries have risen 38% over the past decade alone.

The Reactive Approach

For most seniors today, functional decline is invisible until a crisis makes it visible. A fall triggers an ER visit. A hospitalization reveals a strength problem that had been quietly developing for months. By the time the system responds, real damage has already been done.

  • Decline goes undetected until an incident occurs
  • Care teams respond to emergencies rather than preventing them
  • Families are blindsided by sudden changes in function
  • Recovery is slower, costlier, and less complete after a fall

The Silver Linings Approach

Ongoing monitoring means we see the trend before it becomes a problem. When a resident's gait velocity slows or grip strength drops between visits, we know about it while there is still time to intervene with exercise, referral, or a care plan adjustment.

  • Quarterly visits build a baseline and track change over time
  • Early warnings give care teams time to act before a crisis
  • Residents and families stay informed every step of the way
  • Prevention is always less costly than treatment

The goal is not to respond to decline. The goal is to see it coming and change course before it arrives.

Built by PTs, driven by a simple belief.

Fall prevention shouldn't start after a fall. Silver Linings was built on that belief. Our licensed Physical Therapists designed a quarterly monitoring program that catches small functional changes early, giving seniors, families, and care teams the insight they need to act before something goes wrong.

We partner with senior living communities, assisted living facilities, and care organizations to deliver structured quarterly visits that are thorough, compassionate, and built to produce real results over time.

The Silver Linings Difference

Proactive, Not Reactive We assess before an incident occurs catching early warning signs of decline while there's still time to intervene.
PT-Led Excellence Every monitoring visit is supervised by licensed Physical Therapists not aides or volunteers.
Individualized Reports Each participant leaves with a printed report scores, red flags, early warnings, and personalized recommendations.
Consistent, Trackable Results Our quarterly program tracks results over time turning each visit into another data point in a longitudinal picture of each resident's health.

Simple to run. Powerful in what it reveals.

Silver Linings arrives at your facility, sets up four PT-supervised stations, and walks every resident through a complete functional monitoring visit. Each quarter builds on the last, creating a clear picture of where your population is trending and who needs attention.

01

Intake & Questionnaire

Participants complete intake forms and the FES or ABC fall-confidence questionnaire to establish subjective baseline data.

02

Functional Testing

Three stations each administer two evidence-based tests covering strength, balance, gait, and endurance.

03

Silver Linings Analysis

Results are scored in real time against age-adjusted norms, generating individual risk levels and personalized recommendations.

04

Printed Report & Debrief

Every participant receives a printed personalized report before leaving scores, red flags, and next steps included.

The Six-Test Monitoring Battery

5× Sit to Stand

Measures lower extremity strength and power a strong predictor of fall risk and functional independence.

Timed Up & Go (TUG)

Assesses mobility, dynamic balance, and fall risk through a standardized timed walking task.

Grip Strength

Handgrip strength correlates with overall muscle mass, frailty, and long-term functional decline.

Gait Velocity

Walking speed is one of the most powerful indicators of health status and fall risk in older adults.

4-Stage Balance Test

Evaluates static balance across progressively challenging stance positions to identify instability.

2-Minute Step Test

Measures aerobic endurance and cardiovascular fitness key components of independent mobility.

Every Resident Leaves with a Report

At the end of every visit, each resident receives a printed, personalized report written in plain language that seniors, families, and care staff can all understand and act on.

  • Individual test scores with normative comparisons
  • Red flag alerts for high-risk findings
  • Early warning indicators for subtle decline
  • Personalized exercise and referral recommendations
  • Longitudinal trends tracked visit over visit
Silver Linings

"The best time to catch a problem is before it becomes one. That is what we show up to do, every quarter."
