Ongoing Functional Monitoring for Seniors
Silver Linings Senior Services brings ongoing functional monitoring to senior communities. Our licensed Physical Therapists visit quarterly, track each resident over time, and catch small changes before they become serious problems.
Age Gracefully.
Move Confidently.
Always.
The Problem We Are Solving
By 2030, one in five Americans will be 65 or older. That is 71 million people. And yet for most of them, functional decline goes unnoticed until something goes wrong. A fall. A fracture. A hospital stay. A loss of independence that did not have to happen.
For most seniors today, functional decline is invisible until a crisis makes it visible. A fall triggers an ER visit. A hospitalization reveals a strength problem that had been quietly developing for months. By the time the system responds, real damage has already been done.
Ongoing monitoring means we see the trend before it becomes a problem. When a resident's gait velocity slows or grip strength drops between visits, we know about it while there is still time to intervene with exercise, referral, or a care plan adjustment.
The goal is not to respond to decline. The goal is to see it coming and change course before it arrives.
Our Story
Fall prevention shouldn't start after a fall. Silver Linings was built on that belief. Our licensed Physical Therapists designed a quarterly monitoring program that catches small functional changes early, giving seniors, families, and care teams the insight they need to act before something goes wrong.
We partner with senior living communities, assisted living facilities, and care organizations to deliver structured quarterly visits that are thorough, compassionate, and built to produce real results over time.
The Silver Linings Difference
Services & How It Works
Silver Linings arrives at your facility, sets up four PT-supervised stations, and walks every resident through a complete functional monitoring visit. Each quarter builds on the last, creating a clear picture of where your population is trending and who needs attention.
Participants complete intake forms and the FES or ABC fall-confidence questionnaire to establish subjective baseline data.
Three stations each administer two evidence-based tests covering strength, balance, gait, and endurance.
Results are scored in real time against age-adjusted norms, generating individual risk levels and personalized recommendations.
Every participant receives a printed personalized report before leaving scores, red flags, and next steps included.
The Six-Test Monitoring Battery
Measures lower extremity strength and power a strong predictor of fall risk and functional independence.
Assesses mobility, dynamic balance, and fall risk through a standardized timed walking task.
Handgrip strength correlates with overall muscle mass, frailty, and long-term functional decline.
Walking speed is one of the most powerful indicators of health status and fall risk in older adults.
Evaluates static balance across progressively challenging stance positions to identify instability.
Measures aerobic endurance and cardiovascular fitness key components of independent mobility.
At the end of every visit, each resident receives a printed, personalized report written in plain language that seniors, families, and care staff can all understand and act on.
"The best time to catch a problem is before it becomes one. That is what we show up to do, every quarter."
